This study used Multi-Mode Fiber Diffuse Speckle Contrast Analysis (MMF-DSCA) to noninvasively measure human microcirculatory blood-flow signals and to interpret them in an integrated manner in both the time and frequency domains. Specifically, the cortical study investigated prefrontal hemodynamic responses associated with increasing cognitive load, whereas the finger study examined the relationship between peripheral blood-flow regulation and acupuncture-related physiological concepts, particularly manual stimulation at Zusanli (ST36). ST36 is a representative acupoint located on the anterolateral lower leg below the knee, and the reinforcement (RE) and reduction (RD) methods are traditional manual acupuncture techniques that induce different stimulation patterns through differences in needle rotation and retention. In this study, the Blood Flow Index (BFI), a flow-related optical index, the Blood Volume Index (BVI), a blood-volume-related optical index, and the pulsatile and frequency-domain structures of these signals were analyzed together to evaluate physiological information beyond simple mean blood-flow changes.

In the first study, hemodynamic responses during a composite working memory task were compared using 15 mm and 20 mm source–detector separation (SDS) channels placed over the right prefrontal region. The composite task combined the N-back paradigm with mental arithmetic, thereby imposing progressively increasing cognitive load. As workload increased, behavioral accuracy decreased and heart rate (HR) increased. In the time-domain analysis, 15 mm BFI and BVI from both SDS channels showed task-related changes, whereas 20 mm BFI did not show an obvious change based on the representative time series alone. However, in the frequency-domain analysis, the Cardiac, low-frequency oscillation (LFO), and very-low-frequency oscillation (VLFO) power of 20 mm BFI decreased significantly relative to Baseline, and LFO-band suppression was observed consistently across all workload levels. These findings indicate that task-induced modulation that is not clearly visible in mean or representative time-domain waveforms can be detected more sensitively through frequency-domain analysis.
In the second study, autonomic–peripheral microcirculatory responses induced by ST36 stimulation and the Cold Pressor Test (CPT) were evaluated. CPT is a cold-water stress test that induces acute cold stimulation and autonomic responses. Finger BFI, BVI, HR, and heart rate variability (HRV) were analyzed under the control (CON), reinforcement (RE; 보법, 補法), and reduction (RD; 사법, 瀉法) conditions. BFI and BVI showed a common pattern in which they decreased most prominently during T2 and partially recovered during T3. These changes tended to be greater on the ipsi-lateral side, defined as the cold-water-exposed hand during CPT, namely the left hand in the present protocol. In addition, during T1, repetitive fluctuations of BFI, BVI, and inter-beat interval (IBI) synchronized with the needle rotation–retention cycle were observed, and HR showed the most consistent phase-dependent pattern. In the frequency domain, a decrease in the Cardiac component and selective increases in the LFO/VLFO components were observed, suggesting that ST36 stimulation and CPT induced not merely a simple reduction in peripheral blood flow, but also autonomic–vascular oscillatory reorganization.
Taken together, this study evaluated both prefrontal cognitive-load responses and finger peripheral microcirculatory responses using MMF-DSCA-based diffuse optical flowmetry. The results demonstrate that hemodynamic responses can be understood more precisely when BFI and BVI are interpreted together and when both time-domain and frequency-domain features are analyzed simultaneously. MMF-DSCA may serve as a noninvasive physiological tool for detecting not only mean blood-flow changes, but also pulsatile waveform characteristics, autonomic-related variability, and band-specific microvascular regulatory dynamics.|본 연구는 Multi-Mode Fiber Diffuse Speckle Contrast Analysis (MMF-DSCA)를 이용하여 인체 미세순환 혈류 신호를 비침습적으로 측정하고, 이를 시간영역과 주파수영역에서 통합적으로 해석하고자 하였다. 구체적으로, 대뇌피질 영역에서는 인지부하 증가에 따른 전전두엽 혈역학 반응을 평가하였고, 손가락 말초부위에서는 한의학적 침 자극 이론, 특히 족삼리(ST36) 보사법과 혈류 조절 반응 사이의 관련성을 관찰하고자 하였다. ST36은 무릎 아래 전외측 하퇴부에 위치한 대표적인 경혈이며, 보법(reinforcement, RE)과 사법(reduction, RD)은 침의 회전 및 유지 방식 차이를 통해 서로 다른 자극 특성을 유도하는 전통 수기법이다. 본 연구에서는 혈류 관련 지표인 Blood Flow Index (BFI), 혈액량 관련 지표인 Blood Volume Index (BVI), 그리고 이들 신호의 맥동 구조와 주파수 대역별 변화를 함께 분석함으로써 단순 평균 혈류 변화 이상의 생리학적 정보를 평가하였다.
첫 번째 연구에서는 오른쪽 전전두엽 부위에서 15 mm 및 20 mm source–detector separation (SDS) 채널을 이용하여 복합 작업기억 과제 수행 중 혈역학 반응을 비교하였다. 복합 과제는 N-back paradigm과 mental arithmetic을 결합한 형태로 구성되었으며, 작업부하가 증가할수록 behavioral accuracy는 감소하고 heart rate (HR)는 증가하였다. 시간영역 분석에서는 15 mm BFI와 양 SDS 채널의 BVI가 과제 관련 변화를 보인 반면, 20 mm BFI는 대표 시계열만으로는 뚜렷한 변화를 보이지 않았다. 그러나 주파수영역 분석에서는 20 mm BFI의 Cardiac, low-frequency oscillation (LFO), very-low-frequency oscillation (VLFO) power가 Baseline 대비 유의하게 감소하였으며, 특히 LFO-band suppression은 모든 작업부하 수준에서 일관되게 관찰되었다. 이는 시간영역의 평균 또는 대표 시계열에서 명확히 드러나지 않는 인지부하 관련 조절 반응이 주파수영역 분석을 통해 더 민감하게 포착될 수 있음을 보여준다.
두 번째 연구에서는 ST36 침 자극과 Cold Pressor Test (CPT)에 의해 유도되는 자율신경–말초 미세순환 반응을 평가하였다. CPT는 손을 차가운 물에 담가 급성 냉자극 및 스트레스 반응을 유발하는 검사이며, 본 연구에서는 control (CON), reinforcement (RE), reduction (RD) 조건에서 손가락의 BFI, BVI, HR 및 heart rate variability (HRV) 변화를 분석하였다. 그 결과, BFI와 BVI는 T2 구간에서 가장 뚜렷하게 감소하고 T3 구간에서 부분적으로 회복되는 공통적인 양상을 보였으며, 이러한 변화는 CPT 동안 찬물에 담근 손가락, 즉 ipsilateral side에서 더 크게 나타나는 경향을 보였다. 또한 T1 구간 내에서는 침의 회전–유지 주기와 동기화된 BFI, BVI 및 inter-beat interval (IBI)의 반복적 변동이 관찰되었고, HR은 가장 일관된 phase-dependent pattern을 보였다. 주파수영역에서는 Cardiac component의 감소와 LFO/VLFO component의 선택적 증가가 관찰되었으며, 이는 ST36 자극과 CPT가 단순한 말초 혈류 감소만이 아니라 자율신경 및 혈관성 진동 구조의 재조직화를 유도할 수 있음을 시사한다.
종합하면, 본 연구는 MMF-DSCA 기반 확산광 혈류측정법을 통해 전전두엽 인지부하 반응과 손가락 말초 미세순환 반응을 각각 평가하고, BFI와 BVI를 시간영역 및 주파수영역에서 함께 해석하는 것이 혈역학 반응을 보다 정밀하게 이해하는 데 중요함을 보여주었다. 특히 본 연구 결과는 MMF-DSCA가 평균 혈류 변화뿐만 아니라 맥동 파형, 자율신경 관련 변동성, 그리고 주파수 대역별 미세혈관 조절 특성을 비침습적으로 탐지할 수 있는 생리학적 분석 도구로 활용될 수 있음을 제시한다.
